NAME - thank you for your question, and welcome to Wake Forest! Our Student Health Service page on Immunization requirements says the following about new student requirements:
"All new undergraduate and graduate students are required to be up to date with required vaccines (see policy for list). You and your healthcare physician need to complete the Health Information and Immunization form by July 1."
The Health Information and Immunization form details all the immunizations your student will need to have. There is a portion their doctor has to complete with the applicable information on their vaccine history.
Students have the ability to request a medical or religious exemption request if applicable. Note that the current requirements for spring 2022 were that unvaccinated students had to quarantine for 5 days if exposed to COVID (which means missing classes, as those were in-person). I cannot say yet what the requirements will be for fall 2022, but share the spring protocol in case that would be information your student needs as they decide their course of action.
